This form is for use to notify the department about any citizen who is older or disabled and has information that will assist us in assisting them in an emergency situation.

 

The Police Department usually responds with the Fire Department and Ambulance and usually will arrive on scene first. With the information provided we may be able to assist the citizen much more rapidly and more fully.
